I approve and/or give permission to the Veterinary Care Center of Richmond to offer my dog the use of the high-rise bed and/or comfort pad in the boarding facility during my pet's stay

If my dog decided to chew and/or destroy the property of the Veterinary Care Center of Richmond, I acknowledge and understand that I will be held responsible for the damage fee of the comfort pad, which is $40.00, or the high-rise bed, which is $50.00.

I Acknowledge

I agree that upon pick-up of my dog from the Veterinary Care Center of Richmond, I will provide payment of the property in which was damaged/destroyed.
